What to expect after SAPL surgery: a recovery timeline

A general guide to recovery after lymph-sparing liposuction (SAPL) for lipedema — and how physical therapy supports each phase.

When should I start physical therapy after SAPL?

Often within the first week or two, guided by your surgeon’s protocol — and ideally you’ll have started before surgery, too. Early care is gentle and focuses on managing swelling and moving safely, not pushing.

A general recovery timeline

Every surgeon and every body is different, so treat this as a general map — not a prescription.

  • Before surgery (prehab). Baseline measurements, a compression plan, and gentle conditioning so you start recovery from a stronger place.
  • Week 1–2. Rest, wound care per your surgeon, gentle Manual Lymphatic Drainage and easy movement to manage swelling and comfort.
  • Week 3–6. Swelling gradually settles; compression is reassessed and re-fitted as your shape changes; activity is increased step by step.
  • Month 2–3+. Progressive strengthening, return to daily activity and exercise, and continued compression adjustments.
  • Ongoing. Maintenance visits and a home program to protect your result long-term.

How long does SAPL recovery take?

Initial recovery is typically several weeks, but swelling and compression needs keep evolving over months. Recovery is a continuum, not a single finish line — which is exactly why a Prepare · Recover · Maintain plan helps.
